How to Refer a Patient to WV Health Right

WV Health Right has many specialty clinics both on-site and off-site.  In addition to patients using Health Right as their ‘medical home,' outside providers can access these clinics for patients meeting the Health Right eligibility criteria: uninsured and poor.  Our specialty clinics are listed below.  Any provider wanting to refer a patient to Health Right must completely our referral form. This form must be completely filled out in order for Health Right to determine if the patient can enter our system of care.  Fax the referral form with your medical documentation (labs, notes, EKG, etc.) to 304-343-7009, Attention: Angela Settle, Clinic Coordinator.
