All patients who meet income eligibility requirements and do not have a primary care provider can call themselves to schedule an appointment at 304-414-5930 or 304-343-7003. A physician referral is not required for the transfer of all medical care due to loss of insurance and financial hardship. All patients must present proof of income and photo ID at their first visit. Psychiatric care, workers’ comp cases, disability determination, prenatal care, and care for injuries sustained in an accident covered by insurance (i.e. automobile) are not provided at our clinic. Absolutely no controlled drugs are written or dispensed at our clinic.
